慢性再発性多発性骨髄炎(指定難病270)

○ 概要
 
1.概要
原因不明な、無菌性・非腫瘍性の骨・骨髄の炎症性疾患である。病変は単発性あるいは多発性に発症し、急性・慢性・再発性いずれの経過もとり得るが、このうち多発性に発症し慢性・再発性の経過をとる病態を慢性再発性多発性骨髄炎と呼ぶ。症状として骨痛及びその部位に一致した皮膚の熱感と発赤を認める。

3.症状
高熱を呈することは稀であり、倦怠感や局所の疼痛・腫脹などで緩徐に発症することが多い。疼痛は夜間に強く、運動や寒冷暴露により悪化する傾向がある。
 
4.治療法
非ステロイド抗炎症薬(NSAIDS)に対して50~80%の患者が反応すると報告されている。NSAIDSによる反応が不十分である場合にビスフォスホネートの追加治療が行われる。上記治療無効例に対しては抗TNF製剤、抗IL-1製剤の有効例が報告されている。
 
5.予後
長期的には炎症部の骨の成長障害、変形を来す。また関節炎、掌蹠膿胞症や尋常性乾癬、炎症性腸疾患等の合併が比較的多く報告されている。その他、スイート(Sweet)症候群、壊死性膿皮症、仙腸関節炎、硬化性胆管炎などの合併も報告されている。

<診断基準>
 
慢性再発性多発性骨髄炎診断基準
 
1)画像検査所見:単純レントゲン検査で骨融解と骨硬化の混在像を呈し、かつMRI検査で骨・骨髄浮腫の所見を認める(T1 強調画像で低信号、T2強調及びSTIR画像で高信号)。FDG-PETや骨・ガリウムシンチで多発性病変を確認してもよい。
2)組織検査所見:病変部位の骨・骨髄生検で非特異的炎症像があり、生検組織の培養検査もしくはPCR法により細菌・真菌などの感染症が否定される。
3)他の自己免疫疾患・自己炎症性疾患、悪性腫瘍などの関節炎・骨髄炎の原因となる他疾患を除外する。
 
<診断のカテゴリー>
上記の1)~3)の全ての項目を満たす場合、慢性再発性多発性骨髄炎と診断する。
 
 
 
<重症度分類>
下記の(1)又は(2)を満たした場合は重症例とし助成対象とする。
 
(1)骨髄炎持続例
骨髄炎による疼痛が持続する。なお、骨髄炎の診断は単純レントゲン検査又はMRI検査により確認する。
 
(2)合併症併発例
当該疾病とともに、慢性関節炎、掌蹠膿胞症、尋常性乾癬、炎症性腸疾患、スイート(Sweet)症候群、壊死性膿皮症、仙腸関節炎、硬化性胆管炎のいずれかを認める。
 
※診断基準及び重症度分類の適応における留意事項
1.病名診断に用いる臨床症状、検査所見等に関して、診断基準上に特段の規定がない場合には、いずれの時期のものを用いても差し支えない(ただし、当該疾病の経過を示す臨床症状等であって、確認可能なものに限る。)。
2.治療開始後における重症度分類については、適切な医学的管理の下で治療が行われている状態であって、直近6か月間で最も悪い状態を医師が判断することとする。
3.なお、症状の程度が上記の重症度分類等で一定以上に該当しない者であるが、高額な医療を継続することが必要なものについては、医療費助成の対象とする。
